Western Queensland Primary Health Network

We proudly partnered with Western Queensland Primary Health Network in 2024, joining a multidisciplinary team of practitioners with expertise in nursing, aged care, palliative care, public health education and community development to build capacity in the community for end of life conversations and planning, through encouraging involvement in and offering support for, the Dying to Know Campaign.

Along with conversations on end of life, we busted myths about palliative care, promoted membership of Palliative Care Queensland and disseminated information on how to access palliative care pathways.

In every conversation, people had a chance to hear each other’s experience and desires. There was something for everyone.

Nepean Blue Mountains Primary Health Network

Since 2018, we have partnered with Nepean Blue Mountains Primary Health Network to develop a community-based approach to end of life care in the Upper Blue Mountains, Penrith and Hawkesbury region.  The PHN now has a network of health and community connectors, supported by host community organisations acting as ‘Connector Points’ encouraging more face to face conversations. In  2024, we facilitated their Compassionate Community event where people connected with those involved in local initiatives that have made a difference.

Healthy End of Life Planning (HELP)

We are proud to be partnering with the Healthy End of Life Planning (HELP) team at La Trobe University. We share long standing values around taking action to plan well for end of life and recognising the importance of our communities. The HELP App is a free tool that allows us to coordinate support from our network when we (or someone we love) is at the end of life. It’s functions as a Palliative Care expert in your pocket with relevant tips and articles to support your care journey. We are encouraging people to download the HELP App and start connecting with and activating their network as a D2KD campaign action this year!
