Our Care Navigation service model enables our focus to support people, their families and carers to navigate the health and social care systems to access the services they require, when they require them.

About the role

The purpose of this full-time Care Coordinator, Social Work role is to deliver short-term assessment and case work services to range of customers. Potential customers will be referred by their GP with a range of needs and this role will aid the person with navigating the care and support system and to enhance their ability to access the support/s they require.

Working within a teaming framework, the Care Coordinator, Social Work will provide high quality care navigation services by:

• Undertaking a psychosocial assessment to determine need and develop tailor made care plan
• Supporting the person with developing and collecting the appropriate information and material to assist them to access relevant entitlements and services
• Connecting customers with appropriate informal, mainstream and funded supports and building on informal supports.
• Providing advocacy and referral services as required

• Providing people with information, advice and assistance to source appropriate services
• Providing more intense support at critical times;
• Using multiple strategies to build customer’s capability for improved independence
• Resolving points of crisis (with regard to service)

You will bring

• Current AASW Social Worker registration
• NSW Driver’s Licence, Class C
• Understanding and commitment to a wellness approach to service delivery
• Demonstrated knowledge and experience in mental health, aged care, disability, chronic health care
• Demonstrated knowledge and experience in providing safe and effective clinical care tailored to the client’s needs, goals and preferences to optimise their health and well-being
• Understanding of contemporary treatment, support and community development approaches for people with high and complex care needs living in the community
• Detailed understanding and experience of relevant national and state legislation and health and community care service systems and standards
• High level oral and written communication, consultation, research, report writing and networking skills.
• Demonstrated experience in building customer-driven service development initiatives.

• Demonstrated ability to operate independently and professionally with limited supervision.
• Superior clinical care skills and the ability to mentor and
coach colleagues and support workers to deliver safe and effective clinical care
• Capacity to interpret and work within the relevant policy guidelines.
• Comprehensive experience working in a flexible, multi-skilled team using advanced interpersonal, communication and client service skills.
• High level proficiency in the use of technology-based solutions and the use of relevant computer programs (Client Management Systems, O365 Suite)
