The Psychological Side of Community Care is Also Our Focus

Proveda continues to offer a broad range of mental health services to support and enhance the overall wellbeing of individuals, family members, carers and the community.

Our Mental Health team consists of The Way Back Support Service (NSW), Psychosocial Recovery Coaching, Hoarding and Squalor, and the Dementia Alliance Specialist Service. We also support the community through social programs to reduce isolation and promote connection.

We are well placed to help find you what you need through practical assistance, access to local support networks and by helping to build long-term emotional wellbeing.

The Way Back Support Service (NSW)

The Way Back Support Service NSW (The Way Back) is designed to support people who have recently experienced a suicidal crisis or attempted suicide. The Way Back provides non-clinical, practical support in the community for up to three months. Participation is voluntary and support is guided by your needs.

NDIS Psychosocial Recovery Coaching

Psychosocial Recovery Coaching is a National Disability Insurance Scheme (NDIS) support program for participants with a psychosocial disability who require support managing complex challenges in their daily lives.

Hoarding and Squalor

CCNB’s Hoarding Disorder Hub is a highly specialised resource focused on supporting people, their families and the local service system. The model offers short term case management support, community education, access to peer to peer support, and evidence based therapeutic interventions.

Dementia Alliance Specialist Service

The Dementia Advisory Service aims to support people living with dementia and their carers by providing information, advice, and guidance to appropriate services that suit their needs.

What does Suicide Prevention Accreditation Mean?

By The Way Back Support Service (NSW) gaining suicide prevention accreditation, it has had to meet six stringent Suicide Prevention Standards.  Importantly, the suicide prevention standards provide a level of confidence for the community, that programs accredited through the program (owned by Suicide Prevention Australia) are of high quality and provide a consistent standard of care to the people they’re designed to help.

What is the GP Social Work Program?

Proveda offers support to people referred through a General Practitioner (GP) in the northern Sydney region who are facing complex health challenges and need a little extra support to get back on track.

Our GP social work (GPSW) team identify your needs and the factors causing stress and concern, and work with you for up to 12 weeks, to determine a path forward. We link you with external services and organisations who can assist you, or alternatively, we will provide direct assistance. This may be through utilising the expertise of our wider circle of supports such as our aged care, disability or mental health teams, or it may be by helping you navigate or get assessed for government support.

Either way, our social work team, will work with you to identify your major issues and where you need help, and will provide you with the support you need to make life easier.

How Do I Get Referred?

A referral by a GP located in the Northern Sydney Local Geographical Area (LGA) which spans Hunters Hill, Willoughby, Lane Cove, Mosman, Hornsby, Macquarie, North Sydney and the Northern Beaches, is required to access this free service.

GP’s and practice staff can refer patients to the Proveda GPSW team by completing the form on the link below.

We Focus on Your Recovery

Psychosocial Recovery Coaching is a National Disability Insurance Scheme (NDIS) support program for participants with a psychosocial disability who require support managing complex mental health challenges in their daily lives.

Proveda offers experienced recovery coaches who work one-on-one with you to determine your needs and goals, and then build a plan to step out the method and milestones to achieve these outcomes. Our recovery coaches work to support you (or a loved one) to regain control of your life, return to optimal health and wellbeing, and progress towards maximising your potential. This is done using the Recovery-Oriented Practice Framework.

Our Approach to Psychosocial Recovery is Holistic

A recovery-oriented approach represents a movement away from, a primarily biomedical view of mental illness, to a holistic approach to wellbeing that builds on individual strengths.

We focus on the person as an individual and not just their condition. Following this approach, we understand that everyone is different and should be supported in a way that empowers them to make choices that suit their goals and aspirations regardless of their emotional experiences.

Individual Care Coordination

All our Psychosocial Recovery Coaching customers, have their own individual Care Coordinator. This ensures they fully understand you, your challenges, and your goals, and can work with you to achieve them. We also have back-up care coordinators who can assist you should your care coordinator be unavailable.

You are Not Alone

Proveda offers support to individuals and families going through some of life’s toughest challenges. Whether you are, or someone you love, is experiencing suicidal ideation or you are bereaved by the loss of someone from suicide, we are here to support you.

As the Northern Sydney provider of the government funded program ‘The Way Back (NSW)’, we support people who have been hospitalised through attempted suicide or are struggling with suicidal ideation by linking them with helpful services and assisting to build important safety plans.

Through our Wings of Hope program, we also provide support to families bereaved by suicide at a special annual gathering of remembrance.

Proveda is also proud to say that ‘The Way Back’ program we deliver, recently gained Suicide Prevention Accreditation status, which acknowledges that the program is of the highest quality and provides a consistent standard of care to the people it helps.

The Way Back (NSW)

With our extensive experience in delivering mental health programs over many years, Proveda is proud to be working with all local hospitals, community health teams and GPs to provide support if you have presented to hospital following a suicidal attempt.

We are a local partner in delivering this important government program for all people in the Northern Sydney region and with extensive experience in mental health, we are well placed to stand alongside you in your recovery journey.

Our specialist and skilled staff are here to help, along with any assistance needed through our broader Proveda community support programs. We’ll take the time to ensure you are fully and practically supported by us.  Don’ forget, you are not alone.

A Personalised Way Back Coordination Process

This is a tailored program, focused on surrounding you with all the elements of encouragement and support you need. It provides non-clinical, practical support in the community for up to three months, at no cost to you. We will be completely guided by your needs and connect you with relevant services based on your needs, such as community groups and financial or relationship counselling.

Referral to our service is required by a hospital mental health team or other similar pathway.

When you are referred to The Way Back (NSW), one of our care coordinators will work with you for up to three months to develop a personalised support plan which may include:

Encouraging and supporting you following discharge from hospital or a community mental health service.
Working closely with you to develop your own safety plan.
Referring and connecting you to clinical and community-based support services.
Helping you make, keep, and attend appointments with other services.
Supporting you to set and reach lifestyle goals e.g. housing, work, relationships, education.
Keeping you on track via regular phone or text, depending on what you prefer.

Supporting Families Bereaved by Suicide

WINGS (We Inspire New Growth After Suicide) program was founded in 2007, and joined Proveda (formerly CCNB) in 2020.

It supports people and their families that are bereaved by suicide, by reducing isolation and preventing harm. It does this through providing resources, linkages and direct support to those in need.

Unlike The Way Back program, it is not government funded, and support to families is provided directly by Proveda through fundraising efforts and donations. Often support will be of a practical nature, such as assisting with funeral arrangements, providing assistance with daily living (such as child care, shopping, etc), and by providing comfort by talking to family members, helping them to access counselling, liaising with the police, as well as assisting with managing legal issues.

Annual Memorial Gathering to Remember those Loved and Lost

On International Survivors of Suicide Loss Day, a global day of remembrance initiated in the United States and held annually on the Saturday before Thanksgiving, Proveda hosts a memorial gathering at the Balmoral Headland in Mosman, Sydney.

This event provides an opportunity for families to support each other in their grief and remember those loved and lost. It is an opportunity to meet with others who have the same lived experience in a casual and informal setting. Morning tea is provided and the gathering is hosted by an experienced Proveda team.

Hoarding and Squalor

There are many reasons why someone struggles with hoarding and/or accumulating rubbish, and ending up with a very unclean, or unsafe living environment.

These behaviours and conditions might be a result of a number of psychiatric disorders such as depression, obsessive-compulsive disorder, schizophrenia or may even be the result of a physical disorder. Contributing factors may also be due to dementia or another age-related illness, the suffering of a type of trauma, or from the effects of substance abuse.

We do know that living in unhealthy environments has a huge impact on individuals. They can become even more withdrawn and socially isolated, as personal relationships break down. They may fear eviction from rental properties and be more at risk of health problems. Financial problems are also common.
