What is the GP Social Work Program?

Proveda offers support to people referred through a General Practitioner (GP) in the Northern Sydney region who are facing complex health challenges and need a little extra support to get back on track.

Our GP social work (GPSW) team identify your needs and the factors causing stress and concern, and work with you for up to 12 weeks, to determine a path forward. We link you with external services and organisations who can assist you, or alternatively, we will provide direct assistance. This may be through utilising the expertise of our wider circle of supports such as our aged care, disability or mental health teams, or it may be by helping you navigate or get assessed for government support.

Either way, our social work team, will work with you to identify your major issues and where you need help, and will provide you with the support you need to make life easier.

How Do I Get Referred?

A referral by a GP located in the Northern Sydney Local Geographical Area (LGA) which spans Hunters Hill, Willoughby, Lane Cove, Mosman, Hornsby, Macquarie, North Sydney and the Northern Beaches, is required to access this free service.
