SUMMARY:Advanced Care Planning Workshop
DESCRIPTION:Advance care planning involves planning for your future health care. It enables you to make some decisions now about the health care you would or would not like to receive if you were to become seriously ill and unable to communicate your preferences or make treatment decisions. Advance care planning helps to ensure your loved ones and health providers know what matters most to you and respect your treatment preferences. \nThe workshop will be facilitated by local Nurse Practitioner\, Kelly Arthurs of ANDCare. \nLearning topics cover: \n\nWhat is Advance Care Planning and why it is so important to discuss?\nWhat are the most important aspects to consider with Advance Care Planning\nOpportunity to reflect\, have a conversation\, and commence your own Advance Care Planning journey\nWhat to do next?\n\nThis FREE workshop is for all members of the Ryde and Northern Sydney community and morning tea will be provided. \nAll attendees are eligible for a follow up personal consultation appointment with the Nurse Practitioner\, which will take place on Thursday 28th & Friday 29th August in North Ryde. One on One Appointments are available at no cost for those eligible for Medicare. \nHosted by Sydney North Health Network and City of Ryde Libraries\, with ANDCare \nRegistration is free\, and places are limited.
