About the role

The purpose of the Senior Social Worker role is to deliver short-term assessment, case work and support in the Social Work team. The role supports team members who deliver wholistic care and support for people with a range of complexities.

The primary clients will be referred via the GPSW service, with a range of support needs, and this role will aid the person in navigating the care and support systems, and to enhance their ability to access the supports they require.

Working within a team of Social Workers, the Senior Social Worker will provide high quality care and navigation services by:

  • Undertaking a psychosocial assessment to determine need and to tailor a care plan.
  • Supporting the person with developing and collecting the appropriate information and material to assist them to access relevant entitlements and services.
  • Connecting customers with appropriate informal, mainstream and funded supports and building on informal supports.
  • Providing advocacy and referral services as required.
  • Providing clients with information, advice, and assistance to source appropriate services.
  • Providing incidental counselling and support providers.
  • Providing more intense support at critical times.
  • Using multiple strategies to build customer’s capability for improved independence.
  • Resolving points of crisis.

You will bring

  • Tertiary qualifications in social work and eligibility for membership to AASW.  Relevant experience in community development, social work, assessment, and care planning.
  • Excellent negotiation, decision making and advocacy skills including the ability to interact constructively with a diverse range of stakeholders.
  • Detailed understanding of relevant national and state legislation and standards relevant to the operation of Proveda services.
  • Demonstrated experience in psychosocial assessment and care planning.
  • Exceptional relationship building skills with the capacity to engage with and support GPs and their staff.
  • Excellent oral and written communication skills including experience in the preparation of high-level reports.
  • High level systems thinking and process improvement including the application of technology-based solutions and skills in the use of relevant computer programs.
  •  Ability to provide expert advice and education to enhance service delivery and to guide and support others in providing service excellence, with the capacity to provide supervision to other staff and students.
  • Ability to work as part of a multidisciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing. with situations of a novel, complex or critical nature.
  • Demonstrated commitment to ongoing clinical and professional learning.
  • Current driver’s license.
