We are an independent, not-for-profit provider, so our suggestions for your care, are not determined by internal staffing availability or any other bottom-line profit decisions, but simply based on what we feel is the ‘best fit’ for you. Access to a pool of over 250 providers, ensures we can truly find the right people to deliver your support.
We enjoy helping to simplify and navigate the healthcare system and finding the best service options for you. This is what drives us, as we appreciate that it can be very confusing trying to understand the processes to follow.
Our ‘Care Circle’ model, provides you with an informed team of staff to support you and discuss your options about care, how to maximise your funding, or even how to support you to apply for additional funding when needed. The benefit of this, is that even when your key contact might not be available, other members of the ‘Circle’ can help you.
Regardless of how complex your current or impending circumstances are, or if you have any health or wellbeing concerns, nothing is too hard. We will get to know you, and work with you to ensure you’re best placed and comforted with the full knowledge you need, providing you ongoing access to a wider circle of support, more holistic in nature than other options available.

Helping you get the most from your Home Care Package
As an approved Home Care Package provider, Proveda is here to help you get the support you need.
A Home Care Package is an Australian Government program providing funding for support services for older people who want to stay at home for longer. Home Care Packages are designed to help people aged 65 years and over obtain in-home support to stay living as independently as possible.
A Home Care Package provides access to ongoing services to help with a number of everyday tasks, including domestic services, personal care and clinical care. The program can also help provide mobility aids and minor home modifications.
There are two types of government-funded home support, the Commonwealth Home Support Programme (CHSP) which provides entry level support, and then four levels of Home Care Packages (HCP) ranging from the more basic at Level 1, to the more complex at Level 4.
Typically, CHSP services are short-term (refer to our CHSP information page), while a Home Care Package provides longer-term support for more complex or ongoing needs. To find out if you are eligible for a Home Care Package, visit My Aged Care.

We draw upon our experience and additional services such as our volunteers, dementia and carer advice, opportunities for social connections through the Belong Club, and a range of other services, to tailor the right solution for you.
We have been providing support to the elderly since we started in 1994, gaining considerable expertise in how home care packages work, what services are available to you at each level, and how to maximise the funding you have available. Sometimes, we can support you with free services, which means more of your package funding is available to you for other more needed items.
The job of our care coordinators is to identify your needs and make the best use of your funding with the right providers, so that you are well-supported to live independently at home. In most instances, we don’t provide the direct care, but instead we offer independent advice, and select and coordinate the best aged care support services for you from our pool of over 250 providers, to ensure you get the most out of your Home Care Package (HCP) funding.

Fees and Charges
We believe it is important to be open and transparent with our fees and charges. Proveda’s management fees are below the market average and there are absolutely no hidden charges. Helping you get the most from the Commonwealth Home Support Programme
The Commonwealth Home Support Programme (CHSP) is a specially designed entry-level at home support package, that will ensure you remain independent in your home. As an accredited provider, Proveda will ensure that this first step you take, is the right one.
Whether you have already been assessed as qualifying for the CHSP and have your referral codes, or not, Proveda puts you at the centre, and will assist you to ensure that your first home care experience is exactly what you need. You can rely on us to deliver that extra help, with not only a smile, but with a wider circle of support. You’ll get exactly what you want, when you want, by people you can rely on. You’ll have your own key contact as well as the support of a dedicated Care Circle team.
We understand you know what you need best, and how your needs may have now changed. So, our approach is to ensure you get access to our combined experience of over thirty years of delivering services for people in their homes. This is complemented by access to our community programs, such as the Belong Club. We love assisting you to not only simplify the process, but to help you build connections in the community.
We operate independently, with over 250 providers to choose from to deliver your services. This means that we are best placed to tailor the support you need, ensuring you retain full control over how you want it delivered, at every stage.

Dementia and Memory Loss, We Care
Proveda has helped many hundreds of people (and their families) continue to live fulfilling, and healthy lives following a diagnosis regarding memory loss or dementia.
Our organisation has had the benefit of a longstanding and deep commitment to this important area of health and wellbeing. In fact, helping those experiencing memory loss and dementia has been part of our service offering from the very early days.
Not only do we have a depth of experience in supporting all types of people and families managing life with memory loss, onset and progression, but we are able to offer an extra level of personal care by connecting people through our various community programs.
Our Chatter ‘Cino coffee meet ups are hosted by our Dementia specialist who provides advice for carers and people suffering from memory loss. They provide a friendly environment to talk to others facing similar issues. We know that carers need support too, and this is why we have developed casual opportunities for people to meet and chat. Our strong network of volunteers also assists families by providing comforting social support.
If you are concerned about your memory loss, or that of a family member, you can request an assessment by our clinical care team of registered nurses who will assess cognitive health and make suggestions for how to improve a family living environment for a dementia sufferer. Memory loss and dementia can also be associated with other conditions such as depression and can mean people experience harmful thoughts. If you are having these thoughts, it is important to discuss these with your doctor, who may be able to refer you to one of Proveda’s other programs for support.

As with every other service we provide, Proveda will always put you at the centre of everything we do. As skilled practitioners who have experience in supporting people and families prepare and plan for the memory loss of a loved one, Proveda has an exceptional level of practical experience with managing this issue, so everyone in the family is supported and prepared.
Regardless of what stage the memory loss is at, we can assist you by preparing a management plan, with practical steps to support you along the way. We also provide our regular Chatter ‘Cino dementia coffee catch ups and run a range of programs via our Belong Club. We will also keep in touch via telephone and email.
Depending on the circumstances, we work with everyone who wants to be involved in the preparation and planning for what lies ahead. We provide education on the positive steps that can be taken early in the process, to ensure everyone is supported adequately. We understand the importance of having clarity for the important decisions that may come in the future, and we can provide advice on the most appropriate approach. Our program provides practical and ongoing support as memory loss starts to occur and progresses, so that there is a day-to-day local support network.
Where possible, the person who might be starting to experience memory loss will be very involved in the design of the care plan, and everyone impacted will be able to live their life fully, being informed and completely prepared.
Dementia and memory loss can have a significant impact on both the person experiencing it and those who love them, especially as it progresses, but early detection is very valuable. From our experience, we know that by being prepared and putting the necessary steps in place the impact on loved ones can be lessened, and the appropriate supports can be activated.

Clinical Care Assessment, Management and Monitoring
From time to time, having an objective clinical observation at home, can be good for everyone.
There is nothing more worrying than when a loved one is recovering from an incident. Be it an illness, a hospital visit (like surgery), or even a fall or accident at home. There are particular challenges that can occur, and this is especially true as a person ages. We offer greater peace of mind and reassurance for everyone when we conduct a clinical review to determine a person’s current state. This assessment can be organised by a care coordinator in consultation with the clinical care team.
A clinical assessment is done in consultation with you or a family member, and we take all the appropriate steps to adequately assess how to keep you (or a loved one) safe, in the place they call home. Our team will also introduce proactive strategies to help minimise risk and prevent clinical deterioration.
Whether you already have access to a Home Care Package or not, you can be assured that as a trusted partner, we’ll have your best interests at heart. It’s like having another trained pair of eyes available for any given situation, and we find that working with the family is very beneficial when conducting any of the clinical assessments.

What We Do
As with every other service we provide, Proveda will always put you at the centre of everything we do.
Our organisation not only provides services and supports for aged care package customers, but it also assists customers with complex health care needs through its social work program. A GP referral is required to access the program, which provides more complex and specialist assistance for many people and families experiencing various ageing, psychological or physical conditions.
Providing clinical care services is an extension of our care management, as it relates to an older person’s physical or cognitive function, capacity, condition and mental health, and is a crucial extra service we provide.
Information gleaned from a clinical assessment helps inform decisions around required supports, which in turn, helps the Care Circle teams provide the services needed to assist in maintaining independent living at home. Adequate planning and preparation for a safe home living environment is essential and clinical care reviews can help with forward planning.

Supporting Safety at Home Through Technology
Being reassured that you have some level of monitoring, can provide great comfort if you live on your own, or have health conditions that require some oversight. Our Smart Home Solutions are designed to enable you to continue safely living the life you choose in the place you call home.
For people that have experienced falls, or for families concerned about a loved ones that live by themselves, and who may not be as mobile or alert as in younger years, having a level of discreet monitoring can provide a significant amount of comfort and relief. Monitoring may be as simple as wearing a trackable pendant, or alternatively, it may involve having some motion sensors installed in the home that monitor ‘regular’ activity and provide alerts to others when there is a worrying change to normal patterns.
At Proveda, we believe that technology solutions can enable an improved quality of life and enjoyment which is so important when ageing occurs. We are constantly seeking new ways to deliver technology-based solutions to support better health outcomes.
For many higher-level Home Care Package customers, you may even be able to fund your Smart Home Solution from your package.
What We Do
We can provide a specific technology solution (in partnership with Intelicare), to take much of the worry away for you and your family, and enable a reassuring level of health monitoring 24 hours a day, along with support in an emergency.
Your family or carers can receive automatic notifications should your normal patterns of behaviour change, providing an added level of security for you around your wellbeing.
Along with input from people who care about you and know you well, the in-home sensors are installed to monitor your usual patterns of behaviour – sleep, social and household activity, bathroom use, meal preparation, social activity and climate temperatures. Once your regular patterns are established, it works on the basis that any deviation from this will alert someone in your immediate circle of care, who is identified by you as being a primary contact. This entire system enables you, and those closest to you who want to be actively involved in your care, to gain comfort from an additional level (or levels) of oversight on your health.
The system is not intrusive and the coordination of its installation and any questions you may have about how it works, can be directed to the Proveda team. It is also easy to upgrade to a higher level of monitoring if needed over time.
