Helping you get the most from Support at Home

As an approved provider for Support at Home, Proveda is here to help you get the support you need and the care where you belong, at home.

Support at Home is an Australian Government program providing funding for support services for older people who want to stay at home for longer. The program is designed to help people aged 65 years and over obtain in-home support to stay living as independently as possible.

Support at Home provides access to ongoing services to help with a number of everyday tasks, including domestic services, personal care and clinical care. The program can also help provide mobility aids and minor home modifications.

There are two types of government-funded home support, the Commonwealth Home Support Programme (CHSP) which provides entry level support, and then eight levels of Support at Home (SaH) ranging from the more basic at Level 1, to the more complex at Level 8.

Typically, CHSP services are short-term (refer to our CHSP information page), while Support at Home aims to keep older people healthy, active and socially connected to their community. To access Support at Home, apply through My Aged Care either online, or by calling 1800 200 422.

What We Do

Unlike other providers, our model of independent care management means that we can provide you with greater choice and control over who delivers your services.

And because we have a broader service offering, you can access our Wider Circle of Support that grows with you as your needs change over time.

We have been providing support to the elderly since we started in 1994, gaining considerable expertise in how home care packages work, what services are available to you at each level, and how to maximise the funding you have available. Sometimes, we can support you with free services, which means more of your package funding is available to you for other more needed items.

Some of the free additional services and supports you can access with Proveda include: free activities and events through our Belong Club, volunteer assistance, dementia advice, mental health support and clinical care services.

We treat your individual home care needs with a higher level of care and will not only take the legwork out of matching your needs to the best providers and services, but also proactively coordinate and adjust your requirements, if your needs change over time.

We are here to get you the support you need and how you need it, with access to over 200 providers. So, rest assured we can truly find you the right people to deliver your requirements and we will support you through this process. With Proveda, it’s all about what works for you.

When you join us, you are assigned a key person or ‘lead’ within a Care Circle. If, at any time, your key person is unavailable, you have access to additional team members. This ensures there is always someone to help you and you are never left without the support you need and deserve.

 

Support at Home Levels and Funding

Under Support at Home there are 8 levels of funding with level 1 being the most basic level of care and 8 being the highest, The level you are on and the funding you receive will depend on whether you are a new customer under Support at Home or a transitioned customer from a Home Care Package.

If you are a new Support at Home Customer you will be assessed and allocated an appropriate level based on your current needs.

If you are a transitioned HCP customer, you will transfer to a level which equates to the level you were on for HCP.

If your needs change at any time, you can request an assessment and if it is determined you require a higher level, you will be allocated to this higher level when it is available.

You can view the Support at Home levels and budgets in the tables below. Funding amounts for each classification are outlined in the Schedule of Subsidies and Supplements for Aged Care. Funding amounts are indicative and are subject to the number of days in a quarter and indexation revisions.

Your care is managed by a key contact, but you have the additional benefit of being supported by a dedicated aged care team, so you’re never left without someone who can support you or that understands your needs.

SaH Fees and Charges

At Proveda, we remain proudly not-for-profit. Our commitment is to keep prices fair, transparent, and reflective of the real cost of delivering safe, high-quality care in a complex, regulated environment.

The advantage of having Proveda manage your Support at Home services, is you get access to trained and experienced industry specialists who are always on hand to navigate the system from beginning to end. As an independent provider, we have access to a large pool of service providers at a variety of price points helping us to match you with providers who suit your budget.

Previously with the Home Care Package structure, the hourly rate for your services (Service Fees) was set by individual providers, not Proveda or the Government. In addition, you were also charged separately for things like Care Management, Package Management, travel, or other expenses.

Under the new Support at Home Program, many of these charges are required to be included in the service cost, making fees and charges look a little different to what you might be used to.
