Or perhaps, you have a patient that has just received a life-challenging diagnoses and needs social support to manage everyday tasks around the home? Do you have a patient living in the northern Sydney region with complex health needs that needs a little extra support to get back on track? Proveda's social work program offers 12 weeks of support, during which time the team will identify the needs and factors causing stress and concern and work with the patient to determine a path forward. Proveda’s Assistance with Care and Housing (Hoarding and Squalor) program is accessible through My Aged Care and the Commonwealth Home Support Programme (CHSP), as well as through Home Care Package funding (if applicable). It can also be supported alongside Proveda's GP social work service. Proveda is proud to be working with all local hospitals, community health teams and GPs to provide support to patients who are struggling with suicidal ideation or following a suicide attempt.
