What is Social Prescribing?

Social prescribing is a person-centred approach that enables GPs and other healthcare professionals to refer patients to non-clinical services that support their health and wellbeing. Instead of relying solely on medication or clinical interventions, social prescribing connects individuals with community-based programs that foster social connection, purpose, and resilience. 

Many people experience loneliness, stress, or social isolation, factors that significantly impact physical and mental health. Social prescribing offers a pathway to improved well-being by linking patients to meaningful community engagement.
