Connector Program

This program connects trained volunteers called “Connectors” with people over 65 living with a life-changing diagnosis. The goal is to build social networks and help individuals remain at home in their community for longer. ​Connectors identify additional social and practical supports available locally, while Willing Helpers provide day-to-day companionship and assistance. Adapted from the South West Compassionate Communities Network in Western Australia, this model fosters dignity, independence, and community care.

What is Social Prescribing?

Social prescribing is a person-centred approach that enables GPs and other healthcare professionals to refer patients to non-clinical services that support their health and wellbeing. Instead of relying solely on medication or clinical interventions, social prescribing connects individuals with community-based programs that foster social connection, purpose, and resilience. 

Many people experience loneliness, stress, or social isolation, factors that significantly impact physical and mental health. Social prescribing offers a pathway to improved well-being by linking patients to meaningful community engagement.

How Does Social Prescribing Work?

At the heart of this approach is a dedicated link worker, who helps guide individuals toward activities that best match their interests and needs. This personalised support ensures each person has a meaningful experience, fostering a stronger sense of connection and belonging.
