H&I: GP Social Work Program Explained

Join us for an informative online event where we will delve into the details of our GP Social Work Program and Allied Health Social Work (CHSP) Service. The program provides information, advice and guidance; and linkages to support people to meet their goals and stay well and healthy in their chosen community.

During the event, our expert panel of social workers will explain the key aspects of the program, including its benefits, and success stories. You will have the opportunity to gain a deeper understanding of how social work can enhance patient outcomes and improve overall well-being.

Jay has practiced as a social worker for over 5 years and has been with Proveda for over a year. Jay feels that social work allows her to connect with, mentor and help others achieve their goals and over the years it has given her the opportunity to make a positive difference in the lives of others.

Patty Wong

Patty has been working in the GP Social Work Team at Proveda for two years. Her passion is working with Aged Care clients and helping them navigate the different health care systems. Being a social worker enables her to advocate and provide a voice for others as well as link them to appropriate services.
