Compassionate Connector Program Coordinator

Our Care Navigation service model enables our focus to support people, their families and carers to navigate the health and social care systems to access the services they require, when they require them. This service model includes a range of community programs that support people with non-clinical, social support.
About the Role
The purpose of the Community Programs Coordinator is to promote, recruit volunteers and coordinate the Compassionate Connector Program pilot to achieve positive health outcomes for seniors living with life limiting conditions.
The Compassionate Connector Program is an evidence-based program that is a distinctly new way to support people living with life limiting conditions who are at risk of social isolation, by strengthening their social networks.
The Community Programs Coordinator will have a critical role in the success of this pilot which Proveda hopes to scale to other regions in the future. This is a Part-Time (3 days a week) Fixed-Term contract for 12 months.
Key Responsibilities
- Create referral pathways with community and health stakeholders in Hornsby/Ryde/Northern Beaches LGA
- Co-ordinate educational sessions in Hornsby and Ryde LGA
- Effectively recruit and support ‘Connector’ volunteers and recipients
- Promote and Coordinate training of Volunteers
- Assist with data collection and reporting
- Adhere to Proveda’s Quality Management System including the implementation of quality systems and principles of continuous quality improvement
- Deliver in line with the organisation objectives and agreed performance indicators
- Actively promote Work, Health, Safety and Rehabilitation policies, practices and procedures
About You
Expertise:
- Relevant experience in leadership, community development, social work or social sciences. Tertiary qualifications in social work, social sciences, social policy or related discipline may be preferable but not essential
- Demonstrated understanding or and skills in community development
- Demonstrated experience in goal-oriented service delivery with a focus on achieving outcomes
- Comprehensive knowledge of community care and health sector
Must Have’s:
- Live in the Northern Sydney region
- Must be willing to travel to Hornsby/Ryde/Northern Beaches LGA weekly
