Practice Manager
As the Practice Manager, you will lead and support Proveda’s Care Coordination teams across Aged Care and NDIS services, ensuring the delivery of high-quality, customer-focused care. You will provide leadership, coaching and professional supervision, while driving best practice, service consistency, and positive outcomes for customers.
Key responsibilities include
- Leading and developing Care Coordination teams.
- Promoting high-quality, goal-oriented service delivery.
- Monitoring service quality, productivity, customer outcomes and performance.
- Supporting recruitment, onboarding and employee development.
- Driving continuous improvement and change initiatives.
- Managing complaints, incidents and quality activities.
About You
You are an experienced and approachable leader who is passionate about delivering quality services and supporting teams to thrive.
You will bring:
- Tertiary qualifications in Social Work, Social Sciences, Community Services or a related field.
- Strong knowledge of the NDIS and contemporary service delivery practices.
- Leadership experience within aged care, disability, community services, health, or a related sector.
- Strong knowledge of NDIS and Aged Care services and compliance requirements.
- Experience coaching, mentoring and developing employees.
- Strong communication, problem-solving and stakeholder engagement skills.
- Current NDIS worker Screening check & WWCC (or the ability to obtain)

Proveda is one of the largest community-led organisations in the Northern Sydney Region, providing independent and impartial information, advice and guidance services to older people, those with dementia, people with disabilities, severe and persistent mental illness as well as those with complex health needs. We work with people, their families and communities.
Proveda is seeking a dynamic and experienced person to assist people to successfully navigate the care and support system, implement their NDIS & Home Care Plan and to enhance their ability to access the support/s they require.
