Providing a Supportive Environment is Key to Assistance with Care and Housing

At Proveda, we approach every situation and every person with empathy and respect. We know and understand that sometimes life’s challenges can become overwhelming and that having a supportive environment can make all the difference when you’re working towards positive change. Our approach is to focus on building a genuine connection and fostering mutual respect. This approach not only helps to improve living conditions but also enhances overall wellbeing and quality of life.

The Assisted Care and Housing Program typically runs for six (6) months or more, depending on your level of engagement with the support services provided.

Comprehensive Support

Our dedicated Coordinators will provide one-on-one tailored support to help improve living conditions.

Access to Resources

We’ll connect you or a family member/s to external organisations and resources (e.g. mental health services, cleaning services, and housing providers) to ensure holistic support.

Safety & Risk Management

By improving the home environment, the program reduces significant risks to personal safety and reduces the risk of homelessness. It also ensures emergency services can access the premises if needed.

Emotional & Practical Support

Our Coordinators provide both emotional and practical support, helping you and your loved ones navigate the challenges of hoarding and squalor, as they work through the process of improving the home environment.

How Do You Access the Service?

Proveda’s Assistance with Care and Housing (Hoarding and Squalor) program is accessible through My Aged Care and the Commonwealth Home Support Programme (CHSP), as well as through Home Care Package funding (if applicable). If you would prefer to pay privately for this service, please contact us.

Due to the high volume of referrals and interest in the program, all referrals are evaluated based on priority. Priority is determined during the My Aged Care assessment process and is categorized as low, medium, or high depending on the level of risk and urgency.

If you meet the eligibility criteria, a member of our team will contact you to discuss your needs and the support available. This will include scheduling a home visit to complete a Home Environment Assessment. During this visit, you will also be asked to sign a Service Agreement to formally engage with the program. Please note that due to high demand and limited capacity, you may be placed on a waitlist. We appreciate your patience and understanding.

 

Steps We Will Take:

Understand the entire situation, needs, wishes and behaviours
Assess the living environment with the family or carer
Access care and support through evidence-based treatment plans
Provide support to help secure prolonged tenancy (if eviction is a risk)
Develop a recovery and wellness plan to improve lifestyle
Help navigate government systems such as NDIS and My Aged Care
Provide options for social connection and peer-to-peer mutual support.

Key Services Available Through the Program:

Case Coordination and Planning

Clients receive support through individualized care planning and coordination, with a focus on harm reduction, stabilizing tenancy, and improving overall wellbeing. This may involve liaison with landlords, health services, and community supports, as well as development of practical goals tailored to each client’s unique circumstances.

One-off Targeted Clean-Ups

In some cases, the program may fund or coordinate a single clean-up that addresses specific priority areas within the home—such as clearing access for emergency services, resolving fire hazards, or restoring functionality to essential spaces (e.g. bathroom or kitchen). These are assessed on a case-by-case basis and aligned with broader safety and tenancy goals.

Psychosocial Support

The program adopts a trauma-informed, client-cantered approach to explore the underlying factors contributing to hoarding or squalor. Workers provide emotional support, build motivation for change, and assist in connecting clients to longer-term mental health or social services where appropriate.

Advocacy and Tenancy Support

Advocates may assist with navigating housing provider requirements, preventing eviction, accessing health or social services, and ensuring clients' rights are upheld. This support is especially critical in complex, high-risk situations where clients may feel overwhelmed or misunderstood.

Our Service Model

We assign a dedicated Coordinator to work alongside you (the client), working towards goals and ensuring positive outcomes through one-on-one support. This includes taking measures to maintain a safe and comfortable home environment. Our support is holistic and respectful of each person’s unique circumstances and action is taken through a series of purposeful steps, including:

1. Initial Consultation

We meet to understand unique circumstances and challenges. This involves active listening and building trust and rapport.

2. Needs Assessment

We conduct a thorough assessment of needs, identifying safety concerns and areas requiring immediate attention.

3. Goal Setting

We work together to set realistic and achievable goals aimed at improving the home environment. A collaborative approach ensures buy-in and commitment.

4. Action Plan Development

An individualised action plan is created, outlining specific steps and measures to be taken to work towards goals.

5. Ongoing Support

We provide continuous one-on-one support to help implement the action plan, including practical assistance, emotional support, and regular check-ins to monitor progress.

6. Collaboration with Others

With consent, utilise external organisations to coordinate support services such as mental health services, cleaning services, or housing providers.

7. Review and Adjustments

The action plan is regularly updated and adjusted as needed.
